Companion and Aide Services
A Nurse in the Family has developed five specialty programs each serving a special client need. Within each one of these programs all seven levels of care can be found depending on the need of the client. For each program specially trained and supervised staff attend to the client.
1. Memory Plus Program, our dementia program.
In the Memory Plus Program we stratify dementia into 3 levels: early, moderate, or late stage dementia. Our Certified Quality Care Specialists (CQCS) are trained specifically in each stage and strategies for success with the client according to their individual care plan.
2. Fresh Air Program, our CHF program.
The Fresh Air Program focuses our Care Givers on the pathophysiology of lung disease and more importantly what getting worse looks like. In this way our CQCS can intervene early with the assistance of our Board Certified Physician in managing the client and reducing hospital admissions.
3. Sweet Heart Program, our diabetes program.
Our Sweet Heart Program focuses on diet and lifestyle chances that positively affect diabetes mellitus. Our CQCS are specifically trained in peripheral neuropathy and peripheral vascular disease and know to look for signs of lower extremity ulceration and infection. Additionally our CQCS are educated in identifying the signs and symptoms of hyperglycemia and in particular diabetic ketoacidosis.
4. Safe Home Program, our fall prevention program.
A Nurse in the Family’s safe home program focuses on fall prevention. Recognizing that falls in the elderly are the leading cause of death in geriatric trauma our CQCS are trained in identifying and removing fall hazards in the home and assisting our clients with safe transfers and ambulation.
5. Freedom in Motion Program, our arthritis program.
Our Freedom in Motion Program focuses on helping our clients with arthritis achieve the greatest degree of mobility possible. Pain control strategies specific to each individual client are developed by our Board Certified Physician who personally instructs and supervises the assigned CQCS.
